Windows 10 Apply cumulative Update to extracted Win10-ISO possible?

Assume I download the base ISO for Win10 v1903 and extract the content into a directory (tree).

Then I download a cumulative Update package for this Win10 version.

How can I apply this Update package to the just extracted ISO Win10 directory tree?

How can I later make an ISO out of this update Win10-directory tree?

Why not just download the latest ISO on keep it on a USB. I do and each time there's a new patch/update I just download it and let it re-write it to the USB.

NTLite is a good option for doing something like this (custom ISO). But yes @bassfisher6522 makes the most sense with that.
